Option 1 - Core & Your Choice of Optional Benefits
With this option you take the Core benefits and then may pick your choice of additional benefits to complete your program. Please note that if you choose the dental benefit as an add on, you must take the extended health component as well.
Your benefit plan consists of two components:
1: CORE BENEFITS:
Life Insurance - $25,000
Accidental Death and Dismemberment - $25,000
Critical Illness - $25,000
Employee Assistance Program - A counseling program to assist with trying situations and personal crises
2: OPTIONAL BENEFITS:
Long Term Disability - A group program providing tax free monthly benefit of 66.67% of your gross monthly wage Eligible claims will be paid to age 65.
Vision - $150 every 24 months for glasses or contacts
Extended Health Care - Prescriptions with a Pay Direct Drug card, chiropractor, massage therapy, physiotherapy, eye exams, travel insurance etc
Dental Care* - Choose basic only or basic and major services $1,500 annual limit per family member
*If you choose Dental you have to take Extended Medical
Option 2 - Disability Benefit Only
Benefit Amount :
- pays 66.67% of gross monthly salary
- tax free benefit until age 65
- includes partial disability
